Please Note

Occasionally, due to the unavailability of the appropriate licensed drugs for pets, human medicines or alternative veterinary drugs may have to be used for the treatment of your pet/s. These drugs will only be used when deemed appropriate and necessary. If you should have any questions please do not hesitate to ask a member of staff.

We are able to supply virtually all POM-V (Prescription Only Medicine - Veterinarian) drugs to our clients, as required, for their pets. However, if you would prefer to obtain your pet's drugs from another supplier, the practice is able to provide private prescriptions for POM-V drugs to animals under our care. (Please note this may not be ideal if your animal is an inpatient or where emergency treatment is required.)

The practice is happy to inform you of the prices of any medication.

Animals requiring repeat prescriptions are reassessed by the vet every 6 months to monitor their progress, although this may vary according to individual circumstances. The standard charge for re-examination is that of a repeat consultation fee.

Terms and Conditions

Records

The practice keeps computer records based on information from this form, the clinical notes regarding your pet and any financial history you may have with the practice. This information is for practice use only, but will be given to pet insurance companies for claims, other veterinary practices (when changing practice) and for debt collection purposes.

Fees

Our normal terms of business are for all payments, including non Pet Plan insured cases, to be made at the time of consultation or on collection of your pet after hospitalisation / operation.

For Pet Plan only insured cases, where a claim is to be made, any excesses must be paid immediately and signed insurance claim forms must by provided for the practice.

Any outstanding fees will be referred to Daniels Silverman Limited debt collectors, and will be subject to a surcharge of 15% plus vat to cover the collection costs incurred.
